Transition housing and support for people with disabilities.

The goal of independent living is a complex task for people with disabilities. The Community Integration Project (CIP) helps people with physical disabilities to adapt to their housing and daily living needs while maintaining their quality of life within their home and community.

Housing is a major barrier for people with disabilities who are trying to make the transition from rehabilitation to independent living. To help them, the CIP offers two specially designed and equipped apartments for short-term tenancy of up to six months.

The CIP is located at St. George's Place in Vancouver (at 14th Avenue and Laurel Street). This modern apartment complex includes a mix of residents with and without physical disabilities. It is conveniently located near Vancouver Hospital and GF Strong Rehabilitation Centre.

The CIP focuses on helping each individual to plan and achieve physical independence and community social integration. Support and services include:

Professional rehabilitation management focused on the individual's goals.
Identifying the individual's network of community support needs.
Providing transitional support for the transfer from rehabilitation to independent housing.
Temporary housing at St. George's place and assistance to secure permanent housing.
Assistance accessing rehabilitation services.
Assistance with necessary skills to hire and monitor personal attendants.
