If your company is holding a flu clinic, your staff will need to fill out the
following forms and bring them to the nurse on the day of the clinic:

For the first time ever, we will be offering the Tetanus Diphtheria immunization at the same time and location as your seasonal workplace flu clinic should you and your employees wish to receive it.

About the flu:
- October to May is flu season. Get vaccinated in time!
- Flu is a highly contagious viral infection of the nose, throat and lungs. It is one of the most severe illnesses of the winter season. Typical flu symptoms include abrupt high fever, chills, a dry cough, headache, runny nose, sore throat, and muscle and joint pain.
- Flu can be severe, lasting 1-2 weeks with residual effects up to one month.
- Flu is spread easily from person to person through the air e.g. coughs or sneezes.
- Yearly immunization is the single, most effective means of preventing flu.
Protect yourself by booking one of our Influenza Immunization Clinics:
- Flu vaccine is 70-90% effective in preventing flu. Vaccine is given in the fall/winter (October through March) - for protection during the "flu season."
- Most people have little or no reaction to the vaccine.
- Flu, if contracted, is usually less severe if vaccination has been done.
- The National Advisory Committee on Immunization recommends annual flu immunization.
- Because the flu virus changes from year to year, it is important to get vaccinated with a new flu shot every year.
About the flu vaccine:
